A bilirubin test is often required if it's thought a baby has jaundice. Parental involvement is important to minimise the trauma of having a sick baby and its effect on bonding.

Neonatal jaundice occurs in 60 of term infants and 80 of preterm infants 1 and is caused by hyperbilirubinaemia that is unconjugated (divided into physiological or pathological) or conjugated (always pathological). Causes of pathological jaundice include newborn infections, bacterial sepsis, viral infections, hemolytic disease, and blood type or Rh incompatibility between mother and newborn.

In cases of pathological jaundice the underlying cause must also be treated. Jaundice is the yellow colouring of skin and sclera caused by the accumulation of bilirubin in the skin and mucous membranes. This is the most serious and rarest form of jaundice and requires urgent and immediate treatment. Some causes of pathological jaundice include: breast feeding, hyprothroidism, blood group incompatibility, rhesus factor disease, infection, Crigler-Najjar syndrome, Hirschsprung disease, G6PD deficiency, congenital choledochocyst, and biliary atresia. If bilirubin levels necessitate it, treatment for jaundice involves phototherapy and/or exchange transfusion of donor blood. When jaundice is persistent (pathological jaundice), it may indicate underlying health conditions that need to be treated. Jaundice often corrects itself by the time a baby is around 2 weeks old when the body produces less bilirubin and the liver is more effective at removing it from the body. Jaundice therefore occurs in most of newborn babies. Their livers are less effective in removing it from the blood. Newborn babies have a high number of red blood cells that are broken down and replaced frequently. Because at some point during the first week after birth almost every newborn has a total serum bilirubin (TSB) level that exceeds 1 mg/dL (17 mol/L), the upper limit of normal for an adult, and 2 of every 3 newborns are jaundiced to the clinicians eye, this type of transient bilirubinemia has been called physiologic jaundice. Physiological jaundice results through two distinct. Physiological jaundice is by far the most common cause and is easily recognisable, typically appearing two to three days after birth in an otherwise well infant and normally resolving by 2 weeks of age. After being processed in the liver, bilirubin is passed out of body in poo. Neonatal jaundice can be broadly categorised into physiological and pathological jaundice. Jaundice is caused by too much bilirubin, a breakdown product of red blood cells.
